The step by step guide shown here is only for customers who bought a policy after August 2009, but all My Health Cash Plan customers are welcome to call us for advice on how to start a claim.

Use our 4-step guide to make a claim
Step one – get the paperwork started…
(Don’t forget, you’ll need to check your documents if you bought a policy before August 2009, as the claims process is slightly different.) An original My Health Cash Plan claim form was included with your terms and conditions booklet, but we’re happy to send another one out if you call us. Or, if you’d like to speed things up, you can download the My Health Cash Plan claim form (PDF 85KB), and print it off.
Step two – complete the form and sign it…
It’s important to complete the form with as much detail as possible about your claim, even if it’s for routine healthcare costs. This means that you may need to have sections of the form filled in by your optician, dentist, hospital, specialist or GP. If you’re in any doubt, you can call us for advice – and don’t forget to sign the form yourself…
Step three – include the original receipts with your paperwork…
We need to see original receipts before we can authorise your claim. It’s a good idea to take a photocopy of them for your own records and, if you feel uncomfortable about posting originals, send them to us by Recorded Delivery. The address you need is printed on the form. If you want to make a note of it now, it’s:

Step four – let us pay the bills…
When we have approved your claim, we’ll send payment (up to the limit of your policy), straight to the account that your premiums come from.
