What does my information pack look like?

If you have an Aviva pension plan, we'll send you two information packs as you get closer to your retirement date. Each of these packs will give you guidance on what you should be doing and thinking about.

Six to four months to go

With several months still to go, we'll send you a pack containing:

• A letter - this is an example as your letter will show your details (PDF 52KB)
• Retiring soon brochure (PDF 407KB)
• Personal information form (PDF 80KB)

You should read the pack carefully, complete the personal information form and return it to us. This will allow us to prepare a completely personalised quote to send you later.

Two months to go

With your retirement just round the corner, you need to make some decisions about how you're going to turn your pension fund into a retirement income. We'll send you a pack containing:

• A letter - this is an example as your letter will show your details (PDF 61KB)
• A quote - this is an example as your quote will show your details (PDF 349KB)
• Your retirement guide (PDF 406KB)
• Your retirement agreement (PDF 162KB)
• Key Features - this is an example of what to expect (PDF 137KB)

You should read the pack carefully and talk to a financial adviser before you make your final decision about what to do next.

I don't understand the personal information form. Can you help me?

The personal information form is just a form that asks you to give us some information about yourself. The more we know, the more we can tailor our quote for a retirement income to your personal circumstances. The form asks you about your age, health and lifestyle. You should complete it as fully and accurately as possible. The first set of questions deal with your personal information, such as your name, address, date of birth and so on. We send out two types of forms. The one you receive will depend on what type of pension plan you have with us. The four page form asks for medical details for both yourself and your spouse, civil partner or dependant. You only need to fill in the information for a second person if you want to provide them with an income too.

Read the questions carefully and answer them in as much detail as possible. The more you can tell us, the more we can tailor your annuity to your situation. Some medical conditions may actually mean that you could get a higher level of income. Your doctor may be able to help you fill in some of the medical details.
