New customer authorisation form

This is an initiative that all insurers and intermediaries will take part in and is a mandatory requirement across the industry for UK domestic health insurance and cash plan business, with effect from 1st August 2016. 

It's important that your clients sign and complete the form and forward a copy to us, so we can take any necessary action. Please note, there will be a short concessionary period in place to move from the old style approach to the new template provided. This will also give you time to implement this change into your business. 

The new process will enhance the standard working practices in which we currently work and will help you obtain your clients authorisation in a more transparent way.
