What does my information pack look like?

If you have an Aviva pension plan, we'll send you two information packs as you get closer to your retirement date. Each of these packs will give you guidance on what you should be doing and thinking about.

Six to three months to go

With several months still to go, we'll send you a pack containing:

You should read the pack carefully, complete the personal information form and return it to us. This will allow us to prepare a completely personalised quote to send you later.

Two months to go

With your retirement just round the corner, you need to make some decisions about how you're going to turn your pension fund into a retirement income. We'll send you a pack containing:

I don't understand the personal information form. Can you help me?

The personal information form is a document we ask you to complete detailing personal, lifestyle and medical information. It is very important that you disclose your lifestyle and medical conditions as this may result in you receiving a greater income in retirement. This is because by understanding all of these factors we are able to produce a tailored annuity quote that more closely reflects your life expectancy. You should therefore complete the personal information form as fully and accurately as possible as the more we know the more we can personalise your quote.

You only need to fill in the information for a second person (spouse, civil partner, dependant) if you want to provide them with an income too.

We can complete your Personal Information Form with you over the phone if you would prefer to call us.

Read the questions carefully and answer them in as much detail as possible. The more you can tell us, the more we can tailor your annuity to your situation. Some medical conditions may actually mean that you could get a higher level of income. Your doctor may be able to help you fill in some of the medical details.
