Our simple, three-step claims process
In general, to make a claim, your employees will simply need to follow these three steps:
Download and complete a claim form (PDF 0.2MB) or call us on 0800 158 5191 and we’ll send a claim form in the post.
The claim form will need to be signed or stamped by the provider of the service(s) or treatment, where requested. The completed form can then be sent to us with all original receipts.
Calls may be monitored and/or recorded.

We’ll process the claim and pay back their eligible costs up to the limits in your company’s policy.

My Health Cash Plan for Groups gives you cash back as a contribution towards the cost of day-to-day routine healthcare bills – things like appointments with your dentist or optician.
It's a benefit that's been given to you by your employer, so the best person to ask about your policy's specific details is your group administrator. In a small company, this may be someone in your administration or finance team. In a larger business, it will usually be a member of your Human Resources department.
