When you get in touch, it would help to have these with you:

The policy number (if you know it)

Contact details for the claimant’s doctor, including a telephone number where possible

Dates of any medical investigations or treatment

Any supporting letters from a GP, specialist or other medical professional

The trust documents (if the policy’s under trust)
