My Health Cash Plan helps you cover the cost of your routine healthcare, with the flexibility to give you choices over how you take your benefits. You can manage your policy and make claims here.
Make a claim
If you bought your policy before 2009, please check your policy documents for information about making a claim. Otherwise, follow these steps to make a claim. Whenever you took out your policy, you can call us for advice at any time.
Complete your claim form
You can download our claim form (PDF 106 KB). It’s important to complete it with as much detail as possible, even if it’s for routine healthcare costs. This means that you might need to have sections of the form filled in by your optician, dentist, hospital, specialist or GP.
Include your receipts
We need to see original receipts before we can authorise your claim. It’s a good idea to take a photocopy of them for your own records and, if you feel uncomfortable about posting originals, send them by recorded delivery.
Let us pay the bills
When we have approved your claim, we’ll send payment (up to the limit of your policy) straight to the account that your premiums come from.

Monday to Friday: 9:00am - 5:00pm
For our joint protection, telephone calls may be recorded and/or monitored and will be saved for a minimum of 5 years. Calls to 0800 numbers from UK landlines and mobiles are free.
